Dental Coverage
U-M Health continues to offer a choice of dental plans to meet the needs of team members and their families.
Highlights: Silver (Base) and Gold (Buy-Up) Plans include the following coverage:
- Posterior composite resin fillings and posterior porcelain crowns/bridges
- Special health care needs benefit for team members or dependents who have specific diagnoses which may require additional cleanings/services throughout the year
- Occlusal guard replacement once every five (5) years
Remember that if you participate in a health care Flexible Spending Account (FSA) or Health Savings Account (HSA), you may be able to use your funds on eligible expenses.
Reminder: The Delta Dental Gold Plan (Buy-Up) covers adult orthodontics ($1,500 lifetime maximum)!
BRONZE DENTAL PLAN HIGHLIGHTS
- EPO Network*
- $0 annual deductible
- $0 annual maximum
- Preventative services – exams/cleaning/X-rays: 100% covered
- Child orthodontics (up to age 19): $2,100 copay
- All other services subject to varying copays
*The EPO Network is a smaller, exclusive network of providers. While the coverage is more robust, we encourage team members to view the EPO Network provider directory.
SILVER DENTAL PLAN HIGHLIGHTS
- PPO & Premier Network
- $0 annual deductible
- $1,000 annual maximum
- Preventative services – exams/cleaning/X-rays: 100% covered
- Child orthodontics (up to age 19): 50% covered up to $1,500 lifetime maximum
- $1,000 annual maximum
- All other covered services covered at 50%, up to annual maximum
GOLD DENTAL PLAN HIGHLIGHTS
- PPO & Premier Network
- $0 annual deductible
- $1,500 annual maximum
- Preventative services – exams/cleaning/X-rays: 100% covered
- Child orthodontics (up to age 19): 50% covered up to $1,500 lifetime maximum
- Adult orthodontics (19 or older): 50% covered up to $1,500 lifetime maximum
- All other services covered at 80%, up to annual maximum
