Spousal access provision
U-M Health has a spousal access provision applicable to union groups under its medical plans. Under this provision, spouses of UM Health-Sparrow union employees that have access to medical insurance coverage through their own employer at a premium of $75 monthly/$900 annually*, MUST enroll in single coverage through their employer plan in order to be eligible under U-M Health’s medical plans. Once enrolled, spouses will be eligible to continue coverage on a secondary basis under U-M Health’s medical plan. If you are enrolling your spouse in medical coverage through U-M Health, you must complete the Spousal Access Verification Form and Request for Exemption (if applicable).
*Exceptions are made for spouses that are only offered a high-deductible health plan at $75 a month or less. Please contact HR for more details.
Medical coverage opt-out bonus
If a union team member is eligible for medical coverage through U-M Health but is enrolled in medical coverage through a company/spouse/parent other than U-M Health (which includes all wholly owned Affiliates), s/he may choose to opt-out of the U-M Health medical insurance. Union team members who opt out of health insurance and complete an Opt-Out form (<- use link to Opt Out form) are eligible to receive a $75.00 monthly opt-out bonus.
